2. Medical Records Packet
- [ ] Symptom timeline (pain, tenderness, heaviness, bruising).

- [ ] Quality-of-life impact statement in your own words.
5. Submission and Follow-Up Tracker
- [ ] Date records were submitted to insurer.
- [ ] Reference number / case number.
- [ ] Follow-up call dates and representative names.
- [ ] Appeals or additional-document request log.
